Create a menu form that can be used to access the forms created for questions 1 to 3, which the user was previously asked to create. It must show each form when the relevant command button is selected. Set this form as the default start up project.

: You have two ways for that

    1) You can use only 2 forms
    a) first one for selection of command buttons
    b) second one to display relevent controls according to the option and set "Visible" to "False" of the controls relevent to other controls.
Do the same with the controls for other options.

    2) You can use 4 forms
    a) first one to display Selection Command Buttons
    b) second one to display against second button's click
    c) third one to display against third button's click
    d) forth one to display against forth button's click

If the solution is not according to your requirment then you can contact me back
    bye

Have you even read your books or paid attention in class? Have you even gone to class? If you don't know how to add buttons to a form then our suggestions are just going to confuse you. If you want help, ask specific questions about how to do something in VB, don't give us your assignments and wait for us to do them for you.
