There are plenty of these out there already, check out
http://www.hotscripts.com/ and there is one called Soupermail that is supposed to be pretty good.
If you want to roll your own you need:-
1) To decide what language to program it in.
2) To get the data entered into the form.
3) To send the mail.
How you do 2 and 3 depends on 1.
