I use a web host that has everything you want(I think) plus more, including free SSL and shopping cart, plus the best FAQs. An account similiar to what you want with mysql and php is $20 per domain however, but no setup fees. They have a great browser based interface that lets you manage your entire account with just a browser. The web host is:
