You might try http://www.boutell.com/zipcodes/ and download the data file, add it to a database, and then pull the lat/long when you query a zipcode. If you don't have a db, you could probably do...
header() is a really picky function; if you have any browser output at all before calling it, you won't get anything - that includes any files that you "require" or "include". Check that first....
